Abstract


The quality of performance of an organization or individual can be determined by measuring the performance
on the achievement of quality performance indicators too. Required a comprehensive planning performance
indicators, not normative and does not overlap for rewards and punishment. This study was conducted to
determine the quality of performance indicators, ideal measurement for achieving the vision, mission, goals
and objectives as a basis for measuring the performance of both financial performance and non finansial.
Philosophy of Six Sigma program is used for determination of quality performance indicators, while the
Balanced Scorecard used to design a performance appraisal system that is built on the vision, mission, goals
and objectives of the balance indicator of financial and non financial performance. Based on the integration of
the different stages of the Balanced Scorecard with Six Sigma activities program on the Define and Measure
phase of the DMAIC method obtained 11 (eleven) strategic goals with 22 (twenty two) quality performance
indicators described in the strategy map Department of Cooperatives and SMEs in East Java Province.
Simulation based performance measurement echelon II in 2013, obtained a score of 109.975 with excellent
category (above 100).
